Start your preparation 12-18 months prior to your site visit.

Site Visit Request Form

The information provided in the Site Visit Request Form for Accreditation Site Visit is used for assigning the site visit dates and team.

For Fall continuing accreditation site visits: July 1 (of year prior to visit cycle).
For Fall initial accreditation site visits: January 2 (of year of initial visit).
For Spring continuing accreditation site visits: January 2 (of year prior to visit cycle).
For Spring initial accreditation site visits: July 1 (of year prior to initial visit).

For programs planning an initial site visit, please contact the Candidacy mentor for scheduling details.

For Focused Visits, submit the Site Visit Request Form upon notification that a Focused Visit is required.

Self-Study Report

The Self-Study Report provides a written summary and description of the program’s compliance with all Standards and Criteria. The Self-Study Report is required to initiate the peer review process and provides peer evaluators with an overview of the program as well as the program’s assessment of its compliance with the Standards and Criteria in preparation for the onsite review.

Report Submission

The Self-Study Report and all supporting evidence must be submitted through the ACEN Document Repository.
